If we are not billing your insurance, payment-in-full will be required on the date of service.  If we bill your insurance, you may receive a statement after the visit for any co-pays, deductible, or co-insurance amounts.  We accept the following forms of payment:
  • Cash
  • Check
  • Credit Card (American Express, Discover, Mastercard, Visa)
  • Care Credit
Most credit card payments can be taken over the phone, however Care Credit must be paid either in person, or online.  Care Credit can be used for any balance that is less than six months old; special financing options are available for payments over $200.00.  To learn more or to apply for Care Credit, please visit the Care Credit website.
Insurance Information for Poulsbo Eyecare Center 

When scheduling an appointment, please have your insurance information readily available.  If you are scheduling a routine eye exam (including preventive vision checks or to update a glasses/contact lens prescription), the appointment will be billed as routine vision.  If you are scheduling for a medical reason (including diabetic eye exams, glaucoma screening, and urgent/acute eye issues), the appointment will be billed as medical eyecare.  So, we may ask for both vision and medical insurance information.
 
Ultimately, it is your responsibility to know your insurance and verify which providers are in-network.  The list below is provided as a courtesy, but we encourage you to call your insurance company directly before scheduling to ensure that our office is in-network with your particular group/plan.  You should also check whether your insurance requires a referral from your primary care doctor.

Non-Contracted (Out-of-Network) Insurances 
If your insurance company does not appear on the list above, we are generally not able to bill for your visit or for glasses/contact lenses.  You may have out-of-network benefits, but will be required to pay at the time of service and self-submit a claim for reimbursement.  You should always check with your insurance company to verify out-of-network benefits before scheduling.
