Insurance Information for Poulsbo Eyecare Center
When scheduling an appointment, please have your insurance information readily available. If you are scheduling a routine eye exam (including preventive vision checks or to update a glasses/contact lens prescription), the appointment will be billed as routine vision. If you are scheduling for a medical reason (including diabetic eye exams, glaucoma screening, and urgent/acute eye issues), the appointment will be billed as medical eyecare. We may ask for both vision and medical insurance information when scheduling your first appointment.
Ultimately, it is your responsibility to know your insurance and verify which providers are in-network.
